Requirements
- Completed enrollment application
- Child's up-to-date immunization records
- Georgia Form 3300 (Certificate of Immunization)
- Georgia Form 3231 (Ear, Eye & Dental)
- Copy of birth certificate
- Emergency contact & pickup authorization forms
- Non-refundable registration fee
